It includes essential fields such as the event name, event date, staff member’s name, license, position notes, and the scheduled start and end times of each shift. To support accurate record-keeping, the report also provides blank fields for entering the actual start and end times, and staff signatures. There are three types of Time Sheet Event Reports available-Type A, Type B, and Type C-each varying in the required data fields.
